Preparation for your first appointment

We will need you to please call the office at 7194752229, we will then take your information and provide you with the proper forms needed for your visit.  During our phone call we will ask you to provide your email address and other contact information. Once they are provided, our staff will email the proper forms to you.  

We ask you complete all forms sent to you and return them to the office three days prior to your new patient appointment.  It is also important that you provide us with all testing performed on you and your partner, within the previous year.  These medical records must be sent to us prior to your new patient appointment. This will enable us to provide you a with a comprehensive evaluation without unnecessary delay.  This will enhance the experience for you and the team at RMFC that will be attending to your care.
